- If you do not fill in the required information and save/submit the form, we will be unable to generate a certificate for you.
- After submission you will receive a printable certificate directly to the email address provided on the form within 24 hours.
- DIA is happy to send a copy of your course completion certificate directly to your auto insurance agency. If you would like for us to do that, please provide your agencies email address in the agency portion of this form.
- Both you and your agency will receive a copy of your course completion certificate if you decide to go this route.
